New patient paperwork
• All prescriptions and supplements or nutraceuticals
• All lab results 6-12 months back (and any current)
• Lab work may be acquired prior to your first visit if you do not have anything recent

Does Dr. Lipham treat _____(insert illness here)_____?
Dr. Lipham approaches each individual as a unique case and treats the person not the disease. Therefore, everyone is eligible for a visit!

Is this visit covered by insurance?
If acupuncture or physical therapy are a covered benefit under your insurance plan, you may be eligible for reimbursement. We will furnish you with the required paperwork to begin the reimbursement process. However, payment is due at the time services are rendered. We do not bill insurance companies directly.

How much will this cost?
Please call our office about initial visit pricing as well as reduced rate package options for ongoing care.

What should I wear/bring to Qi Gong?
Qi Gong is meditation through movement, so please wear something light that you are comfortable moving around in. Bring also an attitude of gratitude for your presence practice and the openness to change occurring within the hour.

What should I do to prepare for an acupuncture treatment?
There are no special instructions for preparation other than what we would like you to bring to your initial visit (see above). Hydrate well and eat normally.
Acupuncture is performed with the patient laying face up or face down, if you feel that you will not be able to get up on a treatment table we have seated acupuncture available – please let us know prior to your appointment if you will require an acupuncture chair.

Can a family member come with me?
Absolutely!

What is moxibustion?
Moxibustion or Moxa is a classical Chinese treatment that in ancient times was always paired with acupuncture. For certain individuals it is a primary therapy. It has a sweet and earthy smell and it is safe to inhale.

Does Dr. Lipham sell herbal formulas/supplements to the general public?
Chinese herbal formulas are most effective when they are created specifically for the patient. It is for this reason that we require a consultation before herbs and other products are dispensed.

What is your cancellation policy?
Since new patient appointments are considerably longer than follow-ups, we ask that new patients give us 48hrs notice to avoid a cancellation fee.
Of established patients, we require 24hrs notice to avoid a cancellation fee.
The cancellation fee is 50% of the appointment cost.
We are happy to work with you and arrange rescheduling, just call the office.

Can Dr. Lipham order testing and blood work?
Yes. If you would like to arrange for testing to be done prior to an appointment, we can do that as well. Just give the office a call.

What is HypnoBirthing? Is Dr. Lipham a hypnotist/hypnotherapist?
Dr. Lipham is not a hypnotist or a hypnotherapist. Throughout the training received during the classes you will learn how to guide yourself into a deep state of relaxation. This will allow you to share in the birthing of your child with your partner in a calm and gentle manner.
